APPOINTMENT OF MEDICAL CENTRE OFFERING ITS SERVICES TO THE COMPANY

This is a precedent of an Agreement appointing Medical Centre for offering its services to the employees of the Company at a rate mutually agreed between them and upon the terms and conditions as detailed in the Deed.
