LIVING WILL / ADVANCE MEDICAL DIRECTIVES AND ATTORNEY AUTHORISATIONS

This is a living will and advance medical directives and authorizations by Declarant about steps to be adopted if he/she becomes terminally ill or in a permanent vegetative state with no reasonable expectation of coming back to a reasonable quality of life.
